Tuition Payment Information

  1. Total Program Tuition Fee: $ ____1,075________________

  2. Non-Refundable/ Non Transferable Deposit: $350(This deposit will be applied toward the tuition balance.)

  3. Tuition Payment Schedule:

    • Full Payment due by Week 1 of the program.

    • Payment options:

      •  Full Payment upon Enrollment

      •  $350 deposit full payment by week 1

  4. Payment Due Dates:

    • The $350 non-refundable deposit must be paid upon enrollment to secure a spot in the program.

    • Full payment must be completed by Week 1 of the program. Failure to pay the balance by this time will result in cancellation of enrollment and forfeiture of the deposit.

  5. Late Payment Policy:

    • Payments made after the due date may incur a late fee of $__100______.

    • If full payment is not received by Week 1, the student will not be eligible for an attestation letter to the Ohio Board of Nursing.



Right to Cancel Enrollment

  1. Cancellation Policy:

    • Before Program Start: Students have the right to cancel their enrollment and receive a full refund of tuition paid, minus the non-refundable/non-transferable $350 deposit. Cancellation must be made in writing and received at least 14 days before the program start date.

    • Please be advised that the class may be canceled if the minimum required number of students is not met. In the event of cancellation, your enrollment will be transferred to the next scheduled session, and your deposit will be applied accordingly.

    • After Program Start: After the program begins, students may withdraw but will be responsible for payment based on the following schedule:

      • Withdrawal within the first 2 days: 50% of tuition refunded, minus the non-refundable deposit.

      • Withdrawal after 2 days: No refund will be provided.

  2. Refund Process: Refunds will be issued within 7-10 business days from the date of cancellation or withdrawal request.

  3. Non-Refundable Deposit: The $350 non-refundable deposit will be forfeited under all circumstances except in the case of the program being canceled by the institution.

  4. Eligibility for Attestation Letter to Ohio Board of Nursing:

    • Full payment must be received by Week 1 to be eligible for an attestation letter to the Ohio Board of Nursing. If payment is not completed by Week 1, the student will not be issued this letter.
